Thank you for the answer.

I examined logs and dumps, the config file and logic is OK.

There is a failure_route containing some logic to do
authentication when calling out via VoIP provider
(401 and/or 407 reply codes handling and constructing re-INVITEs
with Authorization header).

For some reason, this VoIP provider sends us 401 reply again and again,
even after we have constructed and sent them our re-INVITE with
Authorization header already. And after 12 attempts the ERROR in log appears.

So, the problem is neither in OpenSIPS, nor in its config/logic but in
VoIP provider.
