Summary: B2B resetting Max-Forwards to 70

Initial Comment:
When using the B2B module in topology hiding mode Max-Forwards appears to get reset to 70 on requests instead of being decremented. The documentation states it should be passed and decremented.

I attached example ngrep output of a request passing through B2B. It is a very simple topology hiding scenario. You can see the Max-Forwards on the INVITE and BYE both get reset.

Hi Ryan,

Actually the doc was not updated according to the last changes in B2B - the
MF hdr is no longer passed from one side to the other, but reset to an
initial value, as any B2B should do - a new call is started, so a fresh MF
values is to be used.

I will take care and update docs.
