  auth_aka: fix async issues reported by Coverity

Defer potentially blocking IPC notifications until user and hash locks
are released, while retaining async parameters across the unlocked
interval. Queue expired requests for dispatch after hash traversal and
initialize AV timestamps before publishing them.

Keep the AV result index stable when consuming retrieval errors to
prevent negative array indexes and invalid remaining counts.
