  dialog: avoid setting last leg without a processing context

Some locally generated replies (timer or locally generated) may run
without a processing context, so only store the last destination leg
when one is available.

  net_tcp: guard connection lookup after shutdown

Late shutdown callbacks may attempt to look up a TCP connection after
tcp_destroy() has released the shared hashes and locks.
