  dialog: match legs by transaction branch

When one outgoing branch receives multiple distinct To-tags, dialog
creates multiple legs for that branch. A subsequent failover branch
could then collide with an existing leg index and trigger the wrongfully
increasing callee_leg warning reported in #3954.
