  OPS-6: Fix some subtle bugs

    * QM_MALLOC + HP_MALLOC with no DBG_MALLOC failed with QM_MALLOC
      because of incorrectly shared "ROUNDTO" constant between .h files
    * bad copy-pasted functions, leading to linker errors
    * undefined NULL symbol
