In simple config, I call b2b_init_request("top hiding") and there are two max-forwards in first INVITE.

Hello,

There were some recent fixes about this, but it seems that one bogus case
got through :)
I have just committed another fix for this both on trunk and 1.8 ( rev 8862
)

Please try it out and see if this happens anymore.
