Summary: Dialplan module broken in 1.8, always "no data in the db"

Initial Comment:
In 1.8.x release, dialplan module never loads records from db. Generated SQL query for loading dialplan results in disabled=NULL, which always returns false. Attached patch fixes this to be disabled=0, thus providing the correct behavior. Patch also fixes typo with disabled_col module param name.
