Initial Comment:
I'd like to use a use_domain setting which works per-module instead of globally. An example scenario where this is useful is:

SIP provider has resellers with many custom domains, yet all users share the namespace so usernames are unique. use_domain is turned off to support that.
Provider wants to support aliases on customer domains, which will be checked before registrations - that allows 123 at provider and 123 at customer-alias to coexist. But without use_domain for aliases alone, it's not possible to easily route allow both 123 at customer1-alias and 123 at customer2-alias.
