Summary: permissions module: address table columns

Initial Comment:
The address table isn't created with the default columns used by the permissions module (mysql).

The problem columns are:

ip_col: the default column name is "ip", however the script creates "ip_addr".

proto_col:  the default for column name is "proto", however this column is not created.

from_col: the default column name is "pattern", however this column is not created.

info_col: the default column name is: "context_info", however this column is not created.

It appears that these columns can be created as char or varchar types with some reasonable length attributes.  The module maintainer would probably know the best type/lengths to use for these columns.

The database schema for the address table has been changed. Try make
dbschema and then create the table again.
