Summary: Error message on db_uri mod init

Initial Comment:
When init module fails because of table version mismatch, a message reporting what database returned vs the version defined for tables within source should be desirable.
Although this happened in db_uri, this could be applied for every modules.
As I have this problem, I made this patch which prints both of the variables when the error appears.
